Username and Password Request Form

If you have misplaced or lost your original username and password for MCSquare, please follow these instructions. First of all, try using the automated Forgot Password page. If that page does not accept your information, you can fill out the form below to request your password to be sent to you. We will send the information to your e-mail account. You can also call our office at 1-800-233-4220 (or 717-796-5039) to request this information.

You must complete the fields checked as required to submit this form. We request your social security number so that we can verify your identity and maintain the security of your personal information available in MCSquare.

Note: If you submit this form after 5:00p.m. EST, your request will be responded to the next business day, Monday through Friday, after 8:00a.m. Upon receiving this request, we will send you the username we originally assigned to you and a new password.
