Job Title: Case Manager

Employer: HealthSouth Rehabilitation of Mechanicsburg

Position Description: Nora works in a physical rehabilitation hospital. Her job entails utilization review (preauthorization, authorization and ongoing review regarding each patient with their insurance companies, coordinating discharge planning referrals and authorizations with approved providers), social service care and referrals for patients while they are in the hospital (contact with family members to bring in clothing or needed items, referrals to the Office of Aging and other outside resources like loan closets for medial equipment, etc.), weekly treatment team meetings with the physiatrist, therapists (physical, occupational, respiratory, and speech), dietician and nurse to review patient progress, goals and current barriers to discharge, problem resolution, patient advocate, and discharge planning (referrals for follow up care, referrals to assisted living or skilled nursing facilities if the patient is unable to return home safely, contact with family or power of attorney to coordinate transportation home, etc.)

Salary Range: Case managers with a bachelor's degree: $20,000 - $35,000; Case manager with a master's degree: $30,000 - $45,000.

Previously Held Positions: Nora has taught summer school through Migrant Education and worked as a Case Manager for the Case Management Unit (formerly Dauphin County Mental Health/Mental Retardation Case Management).

Courses Found Most Helpful: Aging, Child and Adolescent Development, Parenting, and Family Resource Management.

How HDFS Program Helped to Prepare her: The classes in the behavioral science department were all encompassing. I learned many skills that enabled me to truly listen to the needs of others in order to work with them on their level to help them achieve their goals. Since I graduated, I have worked with people from age 0-99 in the human service field. The classes on child development through the aging process have helped me to better understand where some of my clients patients are coming from and what I can do to assist them on their level.
