International Service and Mission Applications

Applications for International Service and Mission Team Trips can be downloaded by clicking on the links below. Fill out the questionaire and email it to Peter Grames at NISMOffice@messiah.edu with "International  Trips Application" in the subject line. Other forms that must be printed and signed should be dropped off at the Agapé Center.

• Health for international travel
• Risk and liability release

 - Alcohol agreement for off-campus
