Missing Receipts

A receipt must be attached to every transaction.  If a receipt was never provided, has not yet arrived or has been lost, use the Missing Receipt Form
  • Complete all sections of the form.
  •  The employee must sign the form.  A supervisor’s signature is NOT required.  
  • If/when a receipt is found / received; it should be attached to the transaction in the IntelliLink system.
