Official academic transcripts are sent upon receipt of the student’s written request.

Enrollment verification can be obtained for insurance, loans, scholarships, etc.

Misplaced or damaged diplomas can be replaced by request.

Notary and Pennsylvania state endorsement of student’s diploma for international service or employment.

Request your academic record be re-evaluated for completion of degree requirements.

Former students wishing to return and complete additional coursework must submit this form.
