• Consultation with RN:  no charge
• Most appointments with Nurse Practitioner:  $20
• Evaluation & Treatment by Physical Therapist:  no charge                  
• Complete History & Physical for college entrance:  $50*
• Pre –Travel Medicine Consultation:  $40**

• Allergy shots:  $10 per week
• Lab-draw fee:  $10
• Rapid strep tests:  $10
• Rapid mono test:  $10
• Urinalysis:  $5
• Ear lavage:  $10
• PPD (Tuberculosis Test):  $8

• Confidential STD testing:  low cost
