Student Verification Form

All verification forms are date stamped with the Registrar's signature and contain the following information: student name, social security number, birth date, current status, all semesters that have been attended, academic standing, estimated graduation date, accreditation, and education code. If a GPA is to be included with the verification, the student must give the Registrar's Office a signed release to do so, which can be found in the Registrar's Office.

Please note: no insurance forms are necessary. Our verification should provide all of the information that you need. However, if your provider mandates the submission of that form, please have the verification sent to you first. You can attach Messiah's verification letter to your original form and submit it to the proper company. Most insurance companies do not require an official college seal, so please feel free to make copies of the form.
