Please fill out this form if you desire to make a housing change at the end of J-term. Forms are due in the Residence Life Office, Eisenhower 207 by Wednesday, January 16, 2013.

 

As this form requires your signature, your current roommate(s) initial(s), your future roommate(s) initial(s) and your RD's signature, please print out this form and bring it to the Office of Residence Life in Eisenhower 207.
