Instructions for Clearance Forms

ALL APPLICANTS MUST SECURE:

Request for Criminal Background Check -can complete and pay online by credit card. Be sure to print the page with the seal as your original clearance.
Pennsylvania Child Abuse History Clearance - Form is online to be printed out, completed, and mailed. See additional instructions below.
FBI Background Checks - Follow instructions for online registration to go to a designated site for electronic fingerprinting. Report your registration number on the Teacher Education Program application as proof of this clearance.

Money orders for child abuse clearances are available at Grantham Post Office for a $.90 fee or at convenience and grocery stores. You must pay cash for a money order. Money orders should be made payable to Department of Public Welfare. Mail the completed form and money order to:
