We have made a number of our forms available online for your convenience. Here are helpful instructions for completing forms. Unless stated otherwise, the forms are in Adobe Acrobat PDF format. If you have questions about these forms or need any other forms please contact us.

I-9 Form

The I-9 Form is a government form used to verify your eligibility for employment. Every new employee must complete this form in person with your supervisor. (See your supervisor for assistance.) As long as you are continually employed by the institution, you are not required to complete this form a second time.

Onboarding Forms

When you are initially hired, you will complete your forms through the Onboarding Journey in Oracle. 

W-4 Form

Residency Certification Form

The Residency Certification form is used to determine your local income tax deduction, which is based on where you live. Any fields that you do not know you may leave blank and the Payroll Office will fill them in. Your PSD Code can be found on this Municipal Statistics page.

Direct Deposit

We encourage all employees to receive their pay through a direct deposit into their bank account. The account may be a checking or savings account.
