Health Forms

 

All full-time students, whether commuting or living on campus, are required to complete ALL student health forms. Failure to complete student health forms in a timely fashion could result in a delay on your arrival day, an academic registration hold for a future semester, or a fine on your student account. Please read the following information carefully so that you can avoid these unnecessary late fees and registration holds.

Important Notes:

  • All health forms are accessible through the Student Health Portal.
  • Health forms will not be accepted via USPS mail, fax or hand-carried. All records are digitally filed.
  • Check your Messiah email and student health portal weekly for messages about forms.
  • Incomplete health forms will cause a delay on your day of arrival on campus.
  • Room keys will not be distributed until ALL forms are completed.
  • If your forms are incomplete: please plan extra time upon arrival to wait in line, receive vaccines, have a physical, and/or fill out forms.
  • If you should have further questions, do not hesitate to call 717-691-6035 or email englecenter@messiah.edu.
  • If you are unable to schedule your physical examination before the May 21 (for fall start students) / December 15 (for spring start students) deadline please contact the Engle Center directly to notify them of the date of your upcoming examination. All other health forms should be submitted by the deadline.

The following forms MUST be completed and submitted via the Student Health Portal no later than May 21 (for fall start students) / December 15 (for spring start students), or promptly after your acceptance if you are admitted after the deadline:

Get started early – This allows time for Engle Center staff to review your documentation and to inform you if anything is incomplete or missing. Sometimes there are problems with medical records, and an early start gives you more time to deal with any issues. You do not have to submit everything at once.

START BY SCHEDULING YOUR PHYSICAL EXAMINATION AND RETAINING A COPY OF YOUR IMMUNIZATIONS. Compare your current immunizations to Messiah's immunization requirements. Go to your Primary Care Provider (PCP) prepared and knowing what you need.

1. Schedule an appointment with your Primary Care Provider (PCP) as soon as possible so that you will have your physical done on time. Some health care providers are scheduled out months in advance. If you are unable to schedule your physical examination before the May 21 (for fall start students) / December 15 (for spring start students) deadline please contact the Engle Center directly to notify them of the date of your upcoming examination.

2. Print out your forms ahead of time and TAKE THEM WITH YOU to your appointment. Be sure to write your NAME AND DATE OF BIRTH on the top of the form.

3. Your physical MUST BE WITHIN 1 year of your start date at Messiah. Athletes - per NCAA regulations, your physical must be within 6 months of your start date.

4. GO TO THE HEALTH PORTAL (PART A FORM) to UPLOAD YOUR COMPLETED FORM.

**ATHLETES -YOU MUST ALSO UPLOAD YOUR FORM TO THE ATHLETIC PORTAL.

The following vaccinations are REQUIRED of all full-time students:

2 MMR vaccines (Measles/Mumps/Rubella)

2 Varicella vaccines OR a Varicella titer showing immune status

2 Meningitis vaccines or 1 within the last 5 years

1 Tdap (TD not accepted) within the last 10 years

Part B requires that you have a SIGNED and VERIFIED copy of your required immunizations from your health care provider AND that you enter the dates of your immunizations into the health portal.

*Messiah University has not mandated COVID-19 vaccination for students or employees. While not mandated, we encourage our student and employee populations be vaccinated.

Some select Messiah University programs which engage in clinical assignments at off campus high risk locations may mandate COVID-19 vaccination (undergraduate nursing, graduate OT/PT).

All vaccination records can be uploaded to the Student Health Portal.

This form will ask you to provide emergency contact and basic demographic information. It will also require a copy of your insurance information.

It will also provide important instructions on how to waive STUDENT HEALTH INSURANCE.

All students enrolled full-time at Messiah University are required to have adequate insurance coverage. We require insurance to ensure that all students are adequately covered in case of an accident or emergency; unfortunately, these are the situations that we cannot plan for. Because ALL students are required to have insurance, ALL STUDENTS ARE AUTOMATICALLY SIGNED UP FOR AND CHARGED for THE SCHOOL INSURANCE (approximately $1106 per semester - for spring start students). After adequate proof of insurance is vetted, the school insurance can be waived. 

Note: Health Insurance is typically available to be waived starting in July (for fall start students ) /  December (for spring start students) who have registered for courses.

This important form provides us with information about your past and current physical and mental health history, your medications and any allergies you might have. Please fill it out honestly and completely, being fully assured of your confidentiality.

While parents/family members can help to provide the student information for this form if needed, it's best to allow the student to complete this form themselves.

Respond to this online screening form by answering each YES or NO question. If all responses are NO or NEGATIVE, you are cleared and require no further testing.

If you have any YES answers, you will need to have a TB test. PRINT THE FORM and take it to your health care provider; have the appropriate TB test; have the form completed by your health care provider and upload the completed form to the Student Health Portal.
