Release Waiver - Each team member must sign the participant waiver.

Release: In consideration of your accepting my entry, I, intending to be legally bound for myself, my heirs, executors, and administrators, do hereby release and discharge Messiah College and the Messiah College Alumni Association, and their respective officers, trustees, agents, and employees, jointly and severally, from any and all liability for illness, injuries , and damages I may suffer arising out of or resulting from my participation in this event. I hereby represent and certify that I am over 18 years of age or have the consent of my parent or legal guardian, that I am in good physical condition, and that I sign of my own free will. THIS IS A RELEASE - READ BEFORE SIGNING. Must be signed by participate and parent/guardian if under 18. I have read over the release waiver and I agree to the terms (please mark one):
