Incomplete health forms will cause a delay on Orientation day**
If your Forms are incomplete: Please plan extra time on orientation day to wait in line, receive vaccines, have a physical, and/or fill out forms.
We are looking forward to partnering with you for a healthy and successful school year! In order to do that well, we need to have you complete your required immunizations, Tuberculosis screening form, and other health forms. This will not only help to ensure your health but also the health of the entire campus community:

These forms are all located on your
health portal. These forms are due on:

PT : August 9, 2021
Dietetics: August 2, 2021
You will receive communication from the Admissions department in emails and newsletters throughout the Spring and Summer, and also emails/secure messages from the Engle Center throughout the Summer advising you of these forms.
A physical exam must be performed by your doctor (within 1 year for all incoming students and within 6 months for students participating in college sponsored sports.) The physical exam form can be found
on the only Engle Center Website under forms and on your health portal. This is the form that we will accept. only
Why do I need a physical exam? We require a physical exam from every student that is studying full time at Messiah. A thorough exam by your doctor ensures that you are healthy enough to attend classes. This information also allows our Health Center Staff to provide you with the best care based on your individual health concerns and needs.
All students MUST complete the
Messiah Tuberculosis Screening Form on the Health Portal. If there are any on the screening form, then you must: YES answers

Yes answers WILL REQUIRE either a PPD skin test OR a blood test to test for TB. You may also require a chest Xray.

**This test must be done even if you have had the BCG vaccine for tuberculosis as a child.
Immunizations and Vaccines
Proof of the following
immunizations are REQUIRED prior to your arrival on campus:
2 MMRs (Measles, Mumps, and Rubella all in one vaccine),
Tdap (Tetanus, Diptheria, and acellular Pertusis) is STRONGLY PREFERRED within the past
5 years; but REQUIRED within the last 10 years Meningitis - 2 meningococcal vaccines (MCV4 or MPSV4) lifetime OR 1 vaccine is acceptable if given in the last 5 years .
2 Varicella vaccines (at least 4 weeks apart).
Immunizations must be:
entered manually entered on the health portal
AND a COPY OF YOUR IMMUNIZATION RECORD from your doctor's office must be uploaded for validation.
Records will only be accepted in English
A NOTE ABOUT THE TDAP VACCINE:
For this academic year only, we are allowing leniency on our Tdap requirement.
All students are still required to have a Tdap (not TD) within 10 years to meet our requirement. While we still recommend updating Tdap before the start of school and believe updating it within 5 years is best practice, our focus must be on encouraging as many students as possible to be vaccinated for COVID 19 prior to the start of the school year so that we can have a "normal" a semester.
A NOTE ABOUT THE COVID 19 VACCINE:
While not currently mandated, it is extremely important that the majority of our student and employee populations be vaccinated prior to the start of the fall 2021 semester. Widespread vaccination of our campus community provides the best opportunity for us to return Messiah to normal, pre-pandemic operations and to make university life feel much more like it is supposed to be for everyone.
A NOTE ABOUT EXEMPTIONS:
We do accept true medical and religious vaccination exemptions in accordance with state law. If you believe you qualify for an exemption, please request a Messiah University vaccination form by emailing
firstname.lastname@example.org. We do not accept exemptions on forms other than our own.
HIPAA and Consent to Treat
HIPAA refers to the Health Insurance Portability and Accountability Act. This is a law that
that provides data privacy and security provisions for safeguarding medical information.
In other words: your private information is safe and only shared with the people who you allow us to.
This form also includes a
Consent to Treat section, that gives us your permission to treat you medically. This form also included a Financial Responsibility section that states you agree to pay for the medical services provided for you.
Signing this form yearly is standard practice in any medical facility in the United States.
The Medical History form provides our Health Center Staff with your specific medical history and details so that we are properly equipped to provide for your individual health needs and concerns. Please include all of your medications, and herbal supplements.
ALL information provided is kept private and confidential
In the case of an emergency, we need to be able to contact your parents/guardians or significant others. Please include different phone numbers and email addresses in case the primary contact cannot be reached.
All students enrolled full time at Messiah University are required to have adequate insurance coverage. We require insurance because we want to ensure that all students are adequately covered in case of an accident or emergency; unfortunately, these are the situations that we don't plan for. Medical bills could really add up, and we don't want students to have to worry about obtaining medical care when needed because of a lack of adequate insurance.
Because ALL students are required to have insurance, ALL STUDENTS ARE AUTOMATICALLY SIGNED UP FOR AND CHARGED for THE SCHOOL INSURANCE (approximately $2282/yr).
After adequate proof of insurance is vetted, the school insurance can be waived.
